ABSTRACT

ObjectiveTo explore the mechanisms of internal treatment (Renshen Baidusan), external treatment (Yurui Enema), and combination of the two methods in treating intestinal mucosal injury in the rat model of ulcerative colitis (UC) from the changes of phosphatidylinositol-3 kinase (PI3K)/protein kinase B (Akt)/nuclear factor-κB (NF-κB) pathway. MethodFifty SPF-grade SD rats were randomized into blank, model, Renshen Baidusan (15.6 g·kg-1), Yurui Enema (25 g·kg-1), and combined treatment (15.6 g·kg-1 Renshen Baidusan + 25 g·kg-1 Yurui Enema) groups (n=10). The rat model of UC was established in other groups except the blank group by 2,4, 6-trinitrosulfonic acid (TNBS)/ethanol. The rats were administered with corresponding drugs once a day for 14 consecutive days since the 8th day after modeling. The histopathological changes of colon were observed by hematoxylin-eosin (HE) staining. Enzyme-linked immunosorbent assay (ELISA) was employed to measure the levels of tumor necrosis factor (TNF)-α, interferon (IFN)-γ, interleukin (IL)-4, and IL-10 in the colon tissue. The apoptosis of colon epithelial cells was detected by terminal deoxynucleotidyl transferase-mediated nick end labeling (TUNEL). The location and expression of B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X protein (Bax), TNF-α, and IL-6 in the colon tissue were examined by immunohistochemistry. Real-time quantitative fluorescence polymerase chain reaction (Real-time PCR) and Western blot were employed to determine the mRNA and protein levels, respectively, of the proteins in the PI3K/Akt/NF-κB pathway in the colon tissue. ResultIn the model group, HE staining showed a large number of inflammatory cell infiltration in the mucosa and submucosa. Compared with the blank group, the model group showed elevated levels of TNF-α and IFN-γ and lowered levels of IL-4 and IL-10 in the colon tissue, increased apoptosis rate of colon epithelial cells, increased positive expression of Bax, TNF-α, and IL-6, and decreased positive expression of Bcl-2 (P<0.05). Moreover, the model group showed up-regulated mRNA levels of PI3K, Akt, and NF-κB and protein levels of PI3K, p-PI3K, Akt, p-Akt, p65, p-p65, Bax, and cleaved Caspase-3, increased Bax/Bcl-2 and cleaved Caspase-3/Caspase-3 ratios, and down-regulated protein levels of NF-κB suppressor protein α(IκBα), Bcl-2, and Caspase-3 in the colon tissue (P<0.05). Compared with the model group, the internal treatment, the external treatment, and the combination (referred to as the three groups) alleviated the colonic mucosal injury, lowered the levels of TNF-α and IFN-γ and elevated the levels of IL-4 and IL-10 in the colon tissue, decreased the apoptosis rate of colon cells, inhibited the positive expression of Bax, TNF-α, and IL-6, and promoted the positive expression of Bcl-2 (P<0.05). Furthermore, the combination group down-regulated the mRNA level of PI3K (P<0.05). The three groups down-regulated the mRNA levels of Akt and NF-κB and the protein levels of p-PI3K, Akt, p-Akt, p65, p-p65, Bax, and cleaved Caspase-3 in the colon tissue, decreased the Bax/Bcl-2 and cleaved Caspase-3/Caspase-3 ratios, and up-regulated the protein levels of IκBα, Bcl-2, and Caspase-3 (P<0.05). ConclusionRenshen Baidusan, Yurui Enema, and their combination may inhibit the activation of PI3K/Akt/NF-κB signaling pathway and regulate the expression of genes and proteins related to this pathway to achieve anti-inflammatory and anti-apoptotic effects, thus restoring the intestinal mucosal barrier function of UC rats.

ABSTRACT

Objective To observe the clinical efficacy of Jiangshabanxia nano-paste on nausea and vomiting in end-stage patients and its effect on the quality-of-life (QOL) in cancer patients. Methods 120 end-stage patients with nausea and vomiting symptoms above grade III were randomly divided into observation group and control group. They were treated with Jiangshabanxia nano-paste and placebo paste respectively. The paste patch was changed every 24 hours and used continuously for 7 days. The nausea and vomiting symptom score, the quality-of-life measurement score and KPS score of cancer patients in the two groups were observed to evaluate the curative effect. Results After 7 days of treatment, the symptom scores of nausea and vomiting in the observation group decreased significantly, the KPS score of the observation group increased, and the effective rate was higher than that in the control group. The score of QOL measurement showed that after treatment, the score of main symptom areas and other symptom areas (except external dyspnea, diarrhea and economic difficulties) in the observation group decreased, and the score of overall health area increased. After treatment, the score of main symptom areas and other symptom areas (except external dyspnea, diarrhea and economic difficulties) in the observation group was lower than that in the control group, and the scores of overall health area in the observation group were higher than those in the control group. Conclusion Jiangshabanxia nano-paste has a good clinical efficacy nausea and vomiting in end-stage patients, it also can improve the quality of life end-stage cancer patients.

ABSTRACT

ObjectiveTo observe the clinical efficacy of compound Huangbai liquid fomentation on diabetic foot ulcer of Wagner grade 1-2. MethodPatients were classified into the observation group and control group, with 41 cases in either group. The observation group received routine therapy and compound Huangbai liquid fomentation, while the control group was treated by routine therapy and medical silver nanoparticles-containing dressing. Ulcer area, ulcer depth, traditional Chinese medicine (TCM) syndrome score, ankle-brachial index, transcutaneous oxygen pressure, vascular endothelial growth factor, epidermal growth factor, advanced glycation end product, high-sensitivity C-reactive protein, interleukin-6, erythrocyte sedimentation rate, and adverse events were observed before and after treatment in two groups. ResultAfter treatment, the ulcer area, ulcer depth, TCM syndrome score, vascular endothelial growth factor, epidermal growth factor, advanced glycation end product, high-sensitivity C-reactive protein, interleukin-6(IL-6), and erythrocyte sedimentation rate were lower than those before treatment in two groups (P<0.05), and ankle-brachial index, transcutaneous oxygen pressure, vascular endothelial growth factor, and epidermal growth factor were higher than those before treatment in two groups (P<0.05). After treatment, the ulcer area, ulcer depth, TCM syndrome score, advanced glycation end product, high-sensitivity C-reactive protein, IL-6, and erythrocyte sedimentation rate were lower in the observation group than in the control group (P<0.05), and ankle-brachial index, transcutaneous oxygen pressure, vascular endothelial growth factor, and epidermal growth factor were higher in the observation group than in the control group (P<0.05). The clinical efficacy of observation group was better than that of the control group (P<0.05). There was no significant difference in the incidence of adverse events and endpoint events between two groups. ConclusionCompound Huangbai liquid fomentation is effective for diabetic foot ulcer of Wagner grade 1-2, as it is beneficial to the healing of the ulcer. This study provides clinical evidence for the further promotion of Chinese medicine fomentation.

ABSTRACT

Objective:To analyze the medication law of external application of TCM in the treatment of psoriasis vulgaris (PV) by using data mining method.Methods:Clinical controlled trial literature about external application of TCM in the treatment of PV was retrieved from CNKI, VIP, WanFang, SinoMed, PubMed and Cochrane Library from the establishment of the databases to May 23, 2022. Prescriptions were extracted after screening. TCM inheritance computing platform V3.0 was used to analyze the property, taste, meridian, efficacy, use frequency, common medicinal pairs and the core combinations.Results:A total 186 prescriptions were included, involving 190 kinds of Chinese materia medica. The medicines were mostly bitter and cold in property and taste, mainly belonging to the liver and heart meridians. Heat-clearing drugs were mainly used, followed by blood circulation-activating and stasis-eliminating medicines, and tonic medicines. The ten most frequently used medicines were Sophorae Flavescentis Radix, Dictamni Cortex, Phellodendri Chinensis Cortex, Kochiae Fructus, Angelicae Sinensis Radix, Salviea Miltiorrhizae Radix et Rhizoma, Poria, Rehmannine Radix, and Arnebiae Radix; the 3 commonly used medicinal pairs were Sophorae Flavescentis Radix- Dictamni Cortex, Sophorae Flavescentis Radix- Cnidii Fructus, and Sophorae Flavescentis Radix- Kochiae Fructus; 4 combinations were obtained through evolution. Conclusions:External application of TCM for the treatment of PV is around the core pathogenesis of "heat toxin", mainly treating from the blood, including cooling blood and detoxifcation, activating blood circulation and removing stasis, nourishing the blood and moistening dryness. Clearing heat and drying dampness, dispelling wind and relieving itching are also valued. The treating thoughts can provide some references for clinical treatment.

ABSTRACT

Traditional Chinese Medicine (TCM) external therapy for sleep disorder of chronic fatigue syndrome (CFS) has good anti-fatigue effect and can improve sleep quality of patients. The treatment for sleep disorders of CFS with TCM external treatment mainly adopts acupuncture, moxibustion, massage, TCM bath, transcutaneous acupoint electrical stimulation and auricular point sticking, etc., or alone, or comprehensive application, or combined with oral Chinese materia medica. The appropriate treatment method can be selected according to the patients' condition and compliance, which reflects the unique advantages of TCM syndrome differentiation and treatment and the treatment according to people and time. The existing research still needs to further form a standardized and recognized diagnosis and treatment system, so as to better guide clinical popularization and application.

ABSTRACT

Objective:To analyze the medication rules in the ancient book Pu Ji Fang for the external treatment of acne based on data mining method. Methods:By screening out the methods of treating acne externally in Pu Ji Fang and establishing a standardized medical record database, this paper adopted the web version of Ancient and Modern Medical Record Cloud Platform to calculate the frequency, properties, flavors, and meridians of those medicines, and conduct cluster analysis by using IBM SPSS Modeler 18.0 software to analyze the association rules. Results:A total of 87 prescriptions were selected, including 164 kinds of Chinese materia medica, among which. Radix Angelicae, Ligusticum Wallichii, Rhizoma Typhoni and lead powder are frequently appeared. The properties of those medicines are mainly warm, cold and mild; the flavors of those medicines are mainly spicy, acrid, sweet and bitter, and the meridians mainly belongs to lung, spleen, stomach and liver meridians. The medical pair and group with the strongest associationion are Ligusticum Wallichii- Radix Angelicae and Rhizoma Typhonii- Radix Angelicae- Ligusticum Wallichii. Those freuently appeared medicines could be grouped into three categories. The paste dosage that was frequently appeared has strong correlation with tallow, mercury and lead powder, while the powder dosage that was frequenctly appeared has strong correlation with Angelica Dahurica, Radix Saponicae, Gleditsia sinensis, Radices Ligustici Sinensis and Ligusticum Wallichii. Conclusions:The application of data mining method could preliminarily reveal the medication rules of Pu Ji Fang for the external treatment of acne. The main treatment method is XinSanFaYue. The three categories of Chinese materia medica are used to treat the syndrome of asthenic habitus attacked by exogenous pathogenic factors, exterior attacked by wind heat and hot blood stasis respectively, showing the rules of treating acne externally before Ming Dynasty and providing references for the clinical treatment of acne.

ABSTRACT

The external treatment of Traditional Chinese Medicine (TCM) in the treatment of recurrent respiratory tract infection for children mainly includes Xiangpei therapy, massage therapy, acupoint application therapy, acupuncture and moxibustion therapy. Many external treatments were combined, or TCM internal and external treatments were combined, which can help to straighten qi and dispel evil and solidify the surface, harmonize viscera qi and blood, dredge meridians, balance yin and yang, tonify lung, spleen and kidney. The treatments can enhance appetite, improve immunity, and help to promote growth. The treatment can help children with recurrent respiratory tract infection improve disease resistance, reduce the number of attacks, alleviate symptoms, shorten the course of disease and improve outcomes, with few obvious adverse reactions. The compliance of children and the satisfaction of family are high.

ABSTRACT

External treatment of Traditional Chinese Medicine (TCM) is a common treatment for lumbar disc herniation, which mainly includes fumigation and washing of TCM, and hot ironing therapy. It can cooperate with oral administration of TCM, Tuina, acupuncture and other therapies to play a synergistic effect and enhance the efficacy. External application of TCM in the treatment of lumbar intervertebral disc herniation is effective with long duration, easy-operated and safe It has showed curative effect in alleviating clinical symptoms and improving lumbar function. Its mechanism mainly includes regulating nerve inflammatory reaction, improving hemorheology and exerting analgesic effect.

ABSTRACT

ObjectiveTo observe the clinical efficacy and safety of traditional Chinese medicine (TCM) external therapeutic protocol of enriching pus for tissue growth (EPTG) in the treatment of Wagner 2-3 diabetic foot ulcer. MethodThe randomized controlled trial (RCT) design was adopted. Patients receiving basic treatment were divided into the EPTG group and the control group (debridement and change of nano-silver medical antibacterial dressing). Ulcer healing rate, ulcer area, ulcer depth, TCM symptom score, visual analogue scale(VAS), transcutaneous partial pressure of oxygen, wound blood flow, inflammatory factors [C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α)], growth factors [vascular endothelial growth factor (VEGF), transforming growth factor-β1 (TGF-β1), and fibroblast growth factor-β (FGF-β)], adverse events, and outcome events of the two groups before and after treatment were observed. ResultCompared with the conditions before treatment, the ulcer healing rate, ulcer area, ulcer depth, TCM symptom score, VAS score, transcutaneous partial pressure of oxygen, wound blood flow, CRP, IL-6, TNF-α, VEGF, TGF-β1, and FGF-β were significantly improved (P<0.01). In terms of the improvement in the ulcer healing rate, ulcer area, ulcer depth, VAS score, transcutaneous partial pressure of oxygen, wound blood flow, CRP, IL-6, TNF-α, VEGF, TGF-β1, and FGF-β, the EPTG group was superior to the control group (P<0.01). There were no statistically significant differences in adverse events and outcome events between the two groups. ConclusionThe TCM external therapeutic protocol of EPTG is safe and effective in the treatment of Wagner 2-3 diabetic foot ulcer. It can greatly reduce the area and depth of diabetic foot ulcer, improve the ulcer healing rate and TCM symptom score, relieve the pain of patients, and improve the microcirculatory blood supply in the local ulcer. Its mechanism of action may be related to the reduction of the local inflammatory response of the wound and the improvement of the proliferation of fibroblasts and vascular endothelial cells.

ABSTRACT

Diabetes and depression are both incurable chronic diseases, which are complementary and integrated. They are clinically called diabetic depression. A large number of epidemiological and clinical studies have confirmed that diabetes interacts with depression, leading to complex conditions of diabetic depression, difficult treatment, and poor prognosis. Diabetes and depression are considered two independent diseases in the majority of the existing clinical methods for the treatment of diabetic depression, with many defects such as low efficiency and severe side effects. Traditional Chinese medicine (TCM) treatment of diabetic depression, characterized by multiple targets, multiple pathways, and overall coordination, can adjust the synergy between multiple systems of the body and is advantageous in the treatment of diabetic depression. However, the evaluation of the efficacy of TCM in the treatment of diabetic depression is mostly limited to the improvement of clinical symptoms, the control of blood sugar level, and the score of depression scale, and there is a lack of in-depth research on the mechanism of action. Therefore, many researchers are devoted to exploring the underlying mechanism of TCM in the treatment of diabetes combined with depression. Clarifying the comorbidity mechanism is the key in treating diabetic depression. In this study, we made an introduction according to the mechanism based on the relevant research reports on the treatment of diabetic depression with TCM and summarized its specific mechanisms, including insulin resistance, oxidative stress and inflammation, and nervous system disorder. The interaction between the various mechanisms has further aggravated the complexity of diabetic depression. We have drawn a diagram of the mechanism pathways of diabetic depression in order to provide new ideas for clinical diagnosis and treatment.

ABSTRACT

Objective:To investigate the effects of external application of modified Xiangsha Liujunzi Tang in improving gastrointestinal reaction,immune function, and quality of life in patients with gastrointestinal tumors after moderately/highly emetogenic chemotherapy. Method:A total of 140 inpatients (from January 2018 to May 2021) with gastrointestinal malignancies diagnosed by the Oncology Department of Integrated Traditional Chinese and Western Medicine of China-Japan Friendship Hospital and treated with moderately/highly emetogenic chemotherapy were randomly divided into an experimental group (<italic>n</italic>=70) and a control group (<italic>n</italic>=70) according to the Good Clinical Practice (GCP). Participants were given routine antiemetic treatment once 20 minutes before chemotherapy (tropisetron 5 mg + dexamethasone 5 mg or methylprednisolone 40 mg). On this basis, the patients in the experimental group were treated with external application of modified Xiangsha Liujunzi Tang for 1 week on the second day after chemotherapy (the selected points were Zhongwan, Neiguan and Zusanli, 6 hours a day, and the application was changed every day), and the patients in the control group were applied with comfort patch at the same time. The gastrointestinal reaction grade, subset concentration of lymphocytes, and Karnofsky score in two groups before and one week after chemotherapy were recorded, and all data were analyzed by SPSS 19.0 statistical software. Result:The grades of chemotherapy-related gastrointestinal reaction in two groups after treatment decreased as compared with that before treatment (<italic>P</italic><0.05). The experimental group was superior to the control group after treatment in terms of grade decrease (<italic>P</italic><0.05). The total response rate of the experimental group was 81.43% (57/70), higher than 62.86% (44/70) in the control group (<italic>χ</italic><sup>2</sup>=9.73, <italic>P</italic><0.05). Stratified analysis was performed on the experimental group. Compared with the conditions before treatment, the grade of gastrointestinal reaction in patients with esophageal cancer and colorectal cancer of the experimental group decreased after treatment (<italic>P</italic><0.05), and that in patients of different genders decreased after treatment (<italic>P</italic><0.05). The results of immune function showed that there was no significant difference in the concentrations of CD3<sup>+</sup>, CD4<sup>+</sup>, CD8<sup>+</sup>, and NK cell subsets of the control group before and after treatment, while the concentrations of CD3<sup>+</sup>, CD4<sup>+</sup>, and NK cell subsets of the experimental group were higher than those before treatment and even superior to those in the control group (<italic>P</italic><0.05). The Karnofsky score of quality of life in the experimental group was higher than that in the control group, with quality of life improved (<italic>P</italic><0.05). Conclusion:External application of modified Xiangsha Liujunzi can improve the gastrointestinal reaction,immune function, and quality of life of patients with gastrointestinal tumors after moderately/highly emetogenic chemotherapy.

It is expected to standardize the clinical application of TCM tinctures, improve the efficacy of this preparation, and reduce its adverse reactions.

ABSTRACT

It is well believed that children's spleen is often insufficient, as the structure and of spleen-stomach are incomplete. The deficiency of spleen-stomach plays an important role in the occurrence, development and pathogenesis of pediatric diseases. Professor has inherited the academic thought of , a famous physician of TCM, and proposed the external treatment method of "supporting the middle". This method emphasizes the constant care of the spleen-stomach of children in the aspect of acupoint selection, and adoptes the external treatment of children's , supporting- cupping therapy (including warming , warming moxibustion and warm scrapping), acupuncture (including abdominal acupuncture, intradermal needling) to treat children's diseases of spleen, lung, kidney and others, which has achieved satisfactory therapeutic effects.

OBJECTIVE@#To observe the clinical efficacy of 's external treatment combined with long-snake moxibustion at the governor vessel for neurogenic bladder after spinal cord injury.@*METHODS@#A total of 64 patients with neurogenic bladder after spinal cord injury were randomly divided into an observation group and a control group, 32 cases in each group. The patients in the control group were treated with routine acupuncture and rehabilitation of bladder function; based on the treatment in the control group, the patients in the observation group were treated with 's external treatment combined with long-snake moxibustion at the governor vessel, twice a week for 8 weeks. Urodynamic test, including residual urine volume (RUV), maximum flow rate of urination (Qmax), bladder pressure at filling phase (Pves), maximum detrusor pressure (Pdet-max) and maximum urinary bladder volume (VMCC), was performed before and after 8-week treatment.@*RESULTS@#The urodynamic indexes in the two groups were improved compared with before treatment (0.05).@*CONCLUSION@#Based on routine acupuncture and rehabilitation of bladder function, 's external treatment combined with long-snake moxibustion at the governor vessel could effectively improve urodynamic indexes, reduce residual urine, reduce bladder pressure and increase the maximum capacity of bladder, thereby improving bladder compliance and bladder function.

ABSTRACT

Objective: To observe the effect of Sanshencao essential oil on the model of thromboangiitis obliterans in rats. Method: Male Wistar rats were randomly divided into five groups according to body weight:sham operation group, model group, Tongsaimai group (1.05 g·kg-1), large and small-dose Sanshencao essential oil groups (0.125 mL·cm-2, twice a day, 0.125 mL·cm-2, once a day), with 10 in each group. The model of occlusive vasculitis was established by injecting sodium laurate solution (10 g·L-1) into the left lower limb artery of rats. Sham operation group was injected with the same amount of normal saline at the same position. Large and small doses of Sanshencao essential oil were applied directly to the affected area, and the remaining groups were given corresponding drugs or pure water for 10 days. One hour after the last administration, the pathological changes were graded; the levels of thromboxane B2(TXB2), interleukin-1 (IL-1), 6-oxo-prostaglandinF1α (6-K-PGF1α), endothelin (ET) were measured by radioimmunoassay. Hematoxylin-eosin(HE) staining was used to observe the pathological changes of the femoral arteries in the left lower limbs. Result: Compared with sham operation group, the degree, extent, inflammation and thrombosis of the affected limbs in model group increased significantly (PPPP2, ET and IL-1 in serum (PP1α (PPConclusion: Sanshencao essential oil has a good therapeutic effect on thrombosis angiitis obliterans model in rats, which related to its anti-oxidation and alleviating inflammation of vascular wall.

ABSTRACT

With the rapid rise of modern drug-derived diseases, the external treatment has received extensive attention at home and abroad, due to its small adverse reactions, convenient application and accurate efficacy.At present, although there are many clinical applications of Chinese medicine external treatment, there are few unified norms and strong arbitrariness in related external treatment techniques, for example, external methods, dosage forms, means, dosage, time, etc.As a result, the development of Chinese medicine external treatment is slow and chaotic, and it can not serve the clinic well.It also hinders the development and expansion of external treatment techniques.This paper discusses the current status of external treatment techniques, and clarifies the development trend of Chinese medicine external treatment, in order to pointing out the direction for the development of Chinese medicine external treatment, it will promote the standardization of Chinese medicine external treatment techniques, and it will be better servicing the clinical practice.

ABSTRACT

Objective: To observe the effect of acupoint application therapy of modified Wuzhuyu Binglang Tang on chronic non-atrophic gastritis (CNAG) and the mechanism of action. Method: Two hundred and seventeen patients were randomly divided into control group (107 cases) and observation group (110 cases) by random number table. Both groups' patients got Omeprazole enteric-coated tablets half an hour before meals, 20 mg/time, 2 times/day, bismuth potassium citrate tablets half an hour before meals, 2 tablets/time, 2 times/day, amoxicillin capsules after meals, 1.0 g/time, 2 times/day, two dimensional furazolidone tablets after meals, 1 tablet/time, 2 times/day, mosapride citrate tablets, 5 mg/time, 3 times/day. The treatment lasted for 2 weeks. Patients in observation group got acupoint application of modified Wuzhuyu Binglang Tang at Zhongwan, Weiyu, Geyu, Shenyu and Piyu for 6 h/day, 1 time/day. And the patients in control group got model drug at the same acupoints. Before and after treatment, epigastric fullness and epigastric pain were scored. Levels of motilin, gastrin, tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and prostaglandin E2 (PGE2) were detected. In 1-month follow-up visit, Hp and adverse reaction were detected under gastroscope. Result: The clinical effect in observation group under gastroscope was better than that in control group (Z=2.015, PZ=2.663, PPχ2=5.002, P2 was higher than that in control group (PConclusion: The acupoint application therapy of modified Wuzhuyu Binglang Tang can relieve the clinical symptoms, promote the negative conversion rate of Hp, improve the clinical effect, regulate gastrointestinal hormones, inhibiting release of inflammatory factors, protect gastric mucosa, and improve gastrointestinal function, with safety in clinic use.

ABSTRACT

Objective To observe the effect of Huangqi granules combined with external treatment on the clinical therapeutic effects of typeⅡand Ⅲ stress injuries. Methods A total of 240 patients with typeⅡ andⅢ pressure injuries admitted to the Hengshui People's Hospital from January 2017 to March 2019 were enrolled. According to difference in therapeutic methods, the patients were divided into astragalus mongholicus granule group and routine treatment of Western medicine group, with 120 cases in each group. In both groups, the patients were given routine nursing treatment such as air cushion bed, regular body turn-over, nutrition support, health education, etc;in routine Western medicine treatment group, according to the principle of aseptic dressing change, the wounds were treated and covered with foam dressing; while in the astragalus mongholicus granule group, the routine nursing care and sterile dressing as above mentioned were also applied, additionally 3 bags of oral astragalus mongholicus granules mixed with boiled water each time, twice a day (equivalent to 10 g for each bag of Chinese herbal slices), 7 days as one course of treatment; at the same time, the wound was sterilized, debrided and washed with normal saline, and after drying, the rubber Shengji ointment for promoting growth of tissue was evenly spread on the wound and covered with foam dressing. In the two groups, the changes of pressure ulcer healing evaluation scale (PUSH) scores before treatment and 7, 14, 21 and 28 days after treatment, as well as the differences in wound healing time and clinical efficacy between the two groups after treatment were observed, and the recurrence rate was followed up for 10 weeks. Results Compared with routine Western medicine group, the Ⅱand Ⅲ wound healing times were significantly reduced in the astragalus mongholicus granule group [the days of wound healing for Ⅱ stress injury (days): 7.81±1.40 vs. 16.52±1.89, the days of wound healing for Ⅲ stress injury (days): 14.60±1.50 vs. 20.23±1.27, both P < 0.05]. With the prolongation of therapeutic time, the PUSH scores of two groups decreased gradually, there was no significant difference in the PUSH scores between the two groups before treatment and 7 days after treatment (both P > 0.05); after 14 days of treatment, the PUSH score of astragalus mongholicus granule group was significantly lower than that of the routine western medicine group (7.82±1.93 vs. 9.96±1.89), and lasted until 28 days (4.16±0.47 vs. 5.29±0.57), the differences being statistically significant (both P < 0.05). The total effective rate of the astragalus mongholicus granule group was significantly higher than that of the routine western medicine treatment group [99.41% (171/172) vs. 74.51% (114/153), P < 0.05], and the recurrence rate of the mongholicus granule group was obviously lower than the routine Western medicine treatment [3.60% (5/139) vs. 17.74% (11/62), P < 0.05]. Conclusion Oral astragalus mongholicus granules combined with myocreatic ointment external therapy can effectively shorten the healing time of type Ⅱand Ⅲ stress injury, improve the cure rate and reduce the recurrence rate.

ABSTRACT

OBJECTIVE:To systematically evaluate the therapeutic efficacy and safety of TCM external treatment for alopecia areata,and to investigate the regularity of prescription selection and medicine use. METHODS:Retrieved from PubMed,EMBase, Cochrane Library,CBM and CNKI,RCTs about TCM external treatment(trial group)vs. routine regimen(control group)in the treatment of alopecia areata were included. The qualities of included studies were evaluated with modified Jadad scale after literature screening and data extraction. Meta-analysis was conducted by using Rev Man 5.3 statistical software. Frequency statistics and ingredient analysis were conduced among included prescriptions. RESULTS:A total of 21 RCTs were included,involving 2 510 patients. Meta-analysis showed that total response rate of trial group was significantly higher than that of control group,with statistical significance [OR=4.14,95%CI(3.04,5.65),P<0.001]. There was no statistical difference in the incidence of ADR between 2 groups [OR=1.66,95%CI(3.04,5.65),P=0.48]. A total of 20 prescriptions among included 21 RCTs were included after the repeated prescriptions were deleted. Above prescriptions were analyzed statistically;there were 53 ingredients in 20 prescriptions,and the sum of use frequency for those ingredients was 138.The ingredients with high use frequency were Carthamus tinctorius,Platycladus orientalis,Salvia miltiorrhiza;main types of medicines were restoring vital energy,relieving exterior disorder,invigorating blood circulation and eliminating stasis,etc. CONCLUSIONS:TCM external treatment shows good therapeutic efficacy and safety. Medicines are mainly used for restoring vital energy,invigorating blood circulation and eliminating stasis.

ABSTRACT

Objective:To evaluate the effect of Hibiscus paste external application combined with traditional Chinese medicine on the inflammatory markers in patients with active rheumatoid arthritis (RA). Methods: The patient information was collected by the data processing system of hospitalized medical records in the First Affiliated Hospital of Anhui University of traditional Chinese medicine. The medical records of patients with rheumatoid arthritis in the Department of Rheumatology from May 2012 to December 2016 were collected. Patients were divided into experimental group and control group according to whether Furong ointment was applied. The random walk model was used to evaluate the influence of Furong ointment combined with traditional Chinese medicine on inflammatory index,erythrocyte sedimentation rate ( ESR) and high sensitivity C reactive protein ( Hs-CRP) in patients with active rheumatoid arthritis. Results: A total of 4 832 patients with rheumatoid arthritis were in accordance with the requirements of the study, including 2 579 in the experimental group and 2 253 in the control group. Baseline data analysis showed that there was no significant difference in general condition, frequency of oral Chinese medicine and core prescriptions between two groups ( P>0. 05 ) . In the experimental group,the maximum ESR of inflammation index was 968,the walking rate was 3 756,the positive growth rate of walking was 0. 257 7,the ratio was 3. 88,the random fluctuation power law value was 0. 364 2±0. 124 6,the comprehensive evaluation index in-creased by 0. 479 9,and the comprehensive evaluation index was 2 017 times. Conclusion: Our hospital rheumatology on the treatment of rheumatoid arthritis from the spleen, oral Chinese medicine prescription to Jianpihuashi drugs, Huoxue Tongluo drugs, Qufengchushi drugs,drug detoxification,two groups of patients with comprehensive evaluation index and acceptable interventions are long-range correlation,a better effect than that of inflammatory markers associated with traditional Chinese medicine in patients with Furong ointment the simple use of Chinese herbs.
